    What happens if you tested positive on initial screening? Is it over now? Not yet. There are many reasons to suspect a false-positive result, which can be due to taking prescribed or over-the-counter medications, human error, etc. Each positive screening should undergo a confirmatory test, which is more sensitive and precise.

    One of the most popular confirmatory test is GC/MS – Gas chromatography/ Mass Spectrometry. This test presents a combination of two analytical techniques, and because of the specific procedure it requires and high costs of it, is always used either for confirmatory tests or for forensic tests for court orders. Unlike drug screens, GC/MS not only detects the drugs presented in a sample, but also measures the quantity, normally in nanograms. This test is effective on all biological fluids, which is very important since blood levels of certain drugs indicate the actual level of intoxication, whereas urine testing cannot determine that.

    GC/MS test works by mixing a sample with organic solvent, which is then evaporated in order to concentrate the drug presence. Gas chromatography forces helium (or other gas) through a thin tube layered with silicone polymer by which the drugs are being separated. Once the drug metabolites have been separated, mass spectrometry identifies the substance – a molecule of certain drugs always break down to unique fragments, always the same, and is compared to a computer library. A 98% match is considered a confirmation of a positive test result. This kind of testing is very precise and not only confirms that presence of toxins in a sample, but also gives an accurate estimate of the levels of any certain substance.

    No employer wants to hire somebody who tested positive for illicit drugs. But what you do in your own time – shouldn’t it be your own business? Unfortunately, big companies can afford to choose its employees, and for a person looking for a job, the choices of where to work might not be as great as the company’s choice of who to hire.
    When you apply for a job first you have an interview and if they are interested in hiring you, you’ll be sent to take a drug test, usually within a short period of time following the interview.
    Most common pre-employment tests are urine tests – they are cheap and give as good results as any other tests. When you submit a urine sample to a lab technician, it is placed in a special tube and marked in front of you and initialed by you, so there is no confusion who’s sample which. Later on approximately half a sample is tested in initial screening. Normally, a positive test results in a person not getting a job, and when they inform you that you weren’t picked for a position, they are not required to let you know why: it might be the drug test results, or it just might be they picked somebody else over you. In case you already have a job and tested positive in initial screening, the company is required to do a second, confirmatory test on the same sample. They do not perform another test, but simply retract the remains of the first sample that is kept in the lab and execute a more advanced test to confirm or disprove the results of the screening.

    We all know there are different types of tests, but what are they and how do they work?
    Most common is an “Immunoassay” testing – it requires the least amount of money a company needs to spend on employee drug testing.
    How does it work:
    Immunoassay is a screening test which gives a positive or negative result. The sensitivity of this test is pretty high. The toxins present in a sample bind with anti-bodies that are created specifically for this kind of testing. The anti-bodies are created by injecting toxins (drugs) into animals like rabbits or sheep. Their immune system produces anti-bodies against any specific toxin; later they are extracted and purified in laboratories so they can be used further in drug screening. When a sample (urine or hair) is combined with these chemical agents, drug metabolites that are present in the sample bind with the anti-bodies thus making a test result positive. If there are no drugs present in a sample, there is no reaction and test comes up negative.
    There are different types of Immunoassay screens: Enzyme- and Radio-immunoassays.
    The first one is based on extracting enzymes from animals’ blood: when an animal is injected with specific drug (or drug metabolite) there are certain antigens that are produced in the blood that bind with that specific drug. After extracting those antigens, there are created into enzymes that are later used to detect drugs in a sample. The degree of reaction between enzymes and drugs metabolites is proportional to the amount of drugs presented in the sample.
    Radioimmunoassay is a similar test that uses radioactive isotopes to detect and measure results of a drug screen.
    Normally, samples that tested positive need to be confirmed by more precise tests.

    Each of the fifty states has their own laws that dictate specific requirements for alcohol and drug abuse in a workplace. Nowadays, home-tests are made easier – a little testing panel gives you a result within minutes, and lets you know whether you are ready or not to pass an official drug test. A simple device, it can dramatically change a person’s life. Make use of the convenience of our time.

    Home drug tests can only show a positive or negative reading. A one-time-use test can check for presence of such toxins as nicotine, alcohol, marijuana, cocaine, opiates, amphetamines, methamphetamines, benzodiazepines, etc. Some panels might include several of toxins that it can test for. If you need to check the exact levels of toxins present in your body, you can go to a private lab, and a simple urine or blood test will tell you how much off you are from a standard cut-off level. If the toxin concentration in a sample is above cut-off level, the sample is considered positive; if the toxin concentration is below cut-off level – the sample is negative. Normally, broad spectrum analysis (first official test that tests for presence of ANY toxins) has the same cut-off level similar to a home-testing device.     - Hello, I have just a few questions about a drug test. It is tomorrow, and I just want to make sure that I’m doing everything right.
    - Ok, what kind of test is it going to be?
    - It is a saliva test, and I’ve been rinsing my mouth with lemon juice and vinegar for a couple of days now, and I also got some mouthwash strips that you use instead of gargling, you know, and just wanted to know when do I put them in?
    - Well, first, lemon juice and vinegar don’t really remove any toxins and is absolutely not necessary to do that unpleasant stuff; secondly, what type of mouthwash strips did you get? Regular mouthwash or special detoxifying one?
    - It’s a regular mouthwash strips, for bed breath, you know… I didn’t know there was a special kind… I’ve been told that lemon juice and vinegar dissolve the mucus that saliva is stored in, and those strips will coat the insides of my mouth with gelatin that works as a barrier between the swab and my mouth…
